Traumatic Brain Injury (TBI)/Polytrauma Case Managers

   

The TBI Polytrauma Case Managers facilitate and coordinate care services that include assessment and evaluation of outpatient referrals for Polytrauma/TBI rehabilitation services for returning service members and all era Veterans. TBI Case Managers work on an interdisciplinary team of providers who offer services that are sensitive and informed to the needs of individuals with a history of traumatic brain injury and complex injuries.  Case Managers provide care management services, coordinate Veteran’s medical care, and screen each Veteran for any related needs.  Case Manager identify  and assist with resources and provide services that help meet the medical, emotional, and psychosocial needs of each Veteran.

Caregiver Support Coordinator and Case Managers

   

Coordinates the application process for the Comprehensive Assistance for Family Caregivers Program, which provides services to family caregivers of eligible Post 9/11 Seriously Injured Veterans.  Point-of-contact for all era family caregivers for education, support, and service referrals.  Organize caregiver focused activities and support groups.  Provide comprehensive case management services to families enrolled in the Post 9/11 program.
